NOVO IN COLLABORATION WITH THE WARRIOR’S HOUSE
As part of the expansion of the terms of cooperation under the signed Memorandum between the NOVO Medical Center and the Municipal Institution of the Lviv Regional Council Warriors House, starting August 1, 2023, we provide free diagnostic services for family members of missing persons under special circumstances.
The services can be used by relatives of the first line of kinship (parents, husband or wife, children, including adopted children).
In order to receive discounted services for MRI, CT, ultrasound diagnostics, endoscopic examinations and consultations of leading specialists, relatives of the missing persons should contact the representative office of the Municipal Institution of the Lviv Regional Council "Warrior's House" at the address: Lviv, T. Bobanycha "Hammera" Street, 1A, or call 098 519 89 20.
In order to participate in our program, you MUST:
1. Receive a referral for a free examination from the program coordinator at the Warrior's House.
2. Call the contact center and pre-arrange the possibility, day and time of appointment according to NOVO's work schedule.
3. Come to us 20 minutes before the appointed time.
4. Provide the NOVO administrator with a passport, an identification code and an extract from the order that the
relative is recognized as a missing person.
If a person under the age of 18 will undergo the examination, the child's birth certificate or passport is have to be attached to the package of documents.
WARNING!
For MRI and CT diagnosis, it is necessary to have a referral from a doctor indicating the area that needs examination.
